Prescott Valley Public Library-New Event-Advance Directives Information and Registration Sessions, August 23rd and 30th, 2024.
Come to the Prescott Valley Public Library on August 23rd for a 30-minute presentation about advance directives from staff of the Arizona Healthcare Directives Registry and return on August 30th to get your advance directive documents filed for free. Advance directives are documents that outline what healthcare and treatment decisions should be made if you are unable to communicate your wishes. Four 30-minute informational sessions will be held throughout the day on Friday, August 23 and will be presented by staff of the Arizona Healthcare Directives Registry. No registration is required for August 23rd; simply drop-in to any 30-minute session. These informational presentations will be held in the Crystal Room on that day from 10:00, 11:00, 1:00 and 2:00. Attend one to learn about: • Living Wills • Healthcare Power of Attorney • Mental Healthcare Power of Attorney • Pre-Hospital Medical Care Directives • The Arizona Health Directives Registry – what it is and what it does Complimentary advance directive forms will be available during these sessions. Come back the following Friday, August 30th, between 10:00 am and 3:00 pm when staff from the Arizona Healthcare Directives Registry (AzHDR) will be on hand to witness and register your completed documents at no cost. Register for a 15-minute time slot on August 30th to have your documents filed. If time allows, AzHDR staff will try to accommodate walk-ins, but this cannot be guaranteed.
